Description
Running anterior to the right main bronchus, the superior right pulmonary vein courses from the hilum toward the left atrium, collecting tributaries from the upper and often middle lobes before forming a short, broad venous trunk. Peripheral venules converge into segmental and lobar veins that track in the intersegmental planes, then pass medially through the root of the right lung alongside the pulmonary artery and the right principal bronchus. Superior to the cardiac base, the trachea descends in the midline and bifurcates into the right and left main bronchi, with cartilaginous rings and the carina rendered as firm landmarks. Coronary arteries and cardiac veins remain visible on the epicardial surface, framing the pulmonary venous entry at the posterior left atrium. Pulmonary veins are often treated as straightforward outflow channels, but their hilar relationships drive real operative risk: the superior pulmonary vein sits anterior and inferior to the pulmonary artery on the right, and variation in middle lobe drainage can place an unexpected venous branch directly in a fissure line during lobectomy. This anterior venous course also matters during thoracoscopic and robotic approaches, where surgeons develop the anterior hilum first to isolate the vein while protecting the phrenic nerve and avoiding traction on the left atrium. Bleeding here is unforgiving. The view offers a clean setup for teaching the difference between pulmonary venous return (oxygenated blood) and systemic venous return, despite the common blue-red color convention used for didactic contrast.
