Description
Positioned at the superior pole of the right lung, the apical vein courses within the upper lobe toward the hilum, where it contributes to the right superior pulmonary vein draining into the left atrium. From an anterior perspective, its tributaries track alongside segmental bronchi and the pulmonary arterial branches, with the vein generally occupying a more anterior and inferior plane at the root compared with the pulmonary artery and main bronchus. Medially, the vessel approaches the mediastinum and pericardial silhouette; laterally, it disappears into the apical parenchyma beneath the pleural surface. Orientation matters. Apical pulmonary venous anatomy is a frequent point of confusion because pulmonary veins do not follow the same anterior posterior relationships as the systemic veins around the superior vena cava, and the right upper lobe pattern varies across individuals. For video-assisted thoracoscopic (VATS) right upper lobectomy and segmentectomy (S1 apical, S2 posterior, S3 anterior), identifying the apical vein and its drainage pattern helps prevent inadvertent division of venous outflow from an adjacent segment, a setup for postoperative congestion or hemorrhage. This anterior view also supports teaching at the hilum, where “RALS” (right pulmonary artery anterior to bronchus, left pulmonary artery superior to bronchus) must be reconciled with the more anterior course of pulmonary veins.
