Description

Presented in an anterior orientation, the cerebrum dominates the superior field with the longitudinal fissure separating right and left hemispheres, while the frontal poles sit most anterior and the temporal lobes fall inferolaterally. Inferior to the cerebral hemispheres, the brainstem (midbrain, pons, and medulla oblongata) aligns on the midline and continues caudally as the cervical spinal cord. From the basal brain, the cranial nerves emerge as paired trunks, with the olfactory bulbs and tracts anteriorly, the optic nerves converging toward the optic chiasm, and the oculomotor, trochlear, trigeminal, abducens, facial, vestibulocochlear, glossopharyngeal, vagus, accessory, and hypoglossal nerves coursing anterolaterally or inferiorly toward their foramina. An anterior cranial nerve view matters when you need to teach where each nerve originates relative to the brainstem and how quickly it enters the skull base. It maps directly onto clinical localization, for example a compressive lesion at the optic chiasm producing bitemporal hemianopia, or a posterior communicating artery aneurysm affecting the oculomotor nerve with ptosis and a dilated pupil.
