Posteriorly, the paired cerebral hemispheres dominate the field, their occipital lobes curving inferomedially toward the midline and separated by the longitudinal fissure. Inferior to the cerebrum, the cerebellar hemispheres and vermis sit beneath the tentorium cerebelli, with folia layered in tight transverse bands. The brainstem lies on the midline, transitioning caudally into the cervical spinal cord, while a yellow-rendered nerve network radiates laterally and inferiorly to suggest the continuity between central nervous system and peripheral nerves. A posterior orientation is the clearest way to teach where the cerebellum actually lives in relation to the occipital cortex and tentorium, a relationship that matters when localizing posterior fossa pathology. Cerebellar infarction or hemorrhage can compress the fourth ventricle and dorsal brainstem and may present with truncal ataxia, nystagmus, and dysmetria rather than the cortical deficits many learners expect from “brain” symptoms. For procedural and imaging correlation, this view also supports discussion of posterior fossa mass effect, tonsillar herniation at the foramen magnum, and why occipital approaches must respect the dural folds and venous sinuses. Use it in neuroanatomy and clinical neuroscience courses to anchor dorsal brain topography, or in medical publishing to accompany sections on posterior fossa stroke, Chiari malformations, and brainstem compression syndromes; the contrasting gray-white CNS and yellow peripheral nerves also works well for patient education on the CNS-to-PNS connection after spinal cord injury.
