Description
Centered within the cranial cavity, the midbrain, pons, and medulla oblongata sit in the posterior cranial fossa, with the pons anterior to the fourth ventricle region and the medulla tapering inferiorly toward the foramen magnum. From a superior view into the skull base, cranial nerves emerge from the brainstem in their expected root entry and exit zones, then course anterolaterally and laterally toward their respective foramina. Yellow nerve bundles radiate toward the internal acoustic meatus, jugular foramen, and hypoglossal canal, while the petrous ridges of the temporal bones form a sharp boundary between middle and posterior fossae. Bony landmarks of the sphenoid and occipital bones frame the central brainstem silhouette. Spatially, this top-down orientation clarifies how neurovascular structures crowd the skull base, a point that matters when correlating brainstem syndromes with cranial neuropathies. A lateral pontine lesion, for example, can explain combined facial paralysis and vestibulocochlear symptoms (CN VII and VIII) near the cerebellopontine angle, while pathology at the jugular foramen region can group dysphonia, dysphagia, and shoulder weakness (CN IX, X, XI). It also helps when teaching why the hypoglossal nerve (CN XII) runs medial and anterior relative to the jugular foramen group, a subtle but clinically testable relationship. Tight quarters.
