Description
Seen in lateral profile, the cerebrum overlies the brainstem, with the midbrain positioned superior to the pons and continuous inferiorly with the medulla oblongata at the level of the foramen magnum. Posterior to the pons and rostral medulla, the cerebellum sits in the posterior cranial fossa, its anterior surface facing the brainstem across the fourth ventricle. The pons forms the prominent anterior bulge of the brainstem in this projection, while the medulla tapers caudally toward the cervical spinal cord. Relative proportions are easy to judge from this angle. A lateral brainstem view matters because it matches how clinicians mentally orient to the posterior fossa on sagittal MR imaging and during approaches to the cerebellopontine angle. The pons and pontomedullary junction anchor common teaching points about cranial nerve exit zones (V from mid-pons, VII and VIII at the cerebellopontine angle, IX to XII along the post-olivary sulcus of the medulla), which is where vestibular schwannoma mass effect and lateral medullary infarction (PICA territory) produce characteristic patterns of deficits. Short. The superior relationship of midbrain to pons also frames the level at which transtentorial herniation can compress the midbrain and alter consciousness and pupillary reflexes.
