Description
Superiorly, the paired cerebral hemispheres frame the midline longitudinal fissure, their cortical gyri and sulci forming the dorsal surface above the posterior fossa. Inferior to the cerebrum, the brainstem occupies the midline, with the midbrain positioned superior to the pons, and the medulla oblongata tapering caudally toward the cervical spinal cord. Posterior to the pons and upper medulla, the cerebellum sits dorsally, its hemispheres flanking the midline vermis and forming the roof of the fourth ventricle via the superior and inferior medullary vela. Spatially, the cerebellum lies posterior to the brainstem, while the brainstem remains anterior to the cerebellar parenchyma at the cerebellopontine and cerebellomedullary junctions. Posterior anatomy of the brainstem matters because it is the operative and imaging corridor for fourth ventricular and dorsal pontomedullary lesions, where millimeters decide whether you injure brainstem nuclei or long tracts. Entry zones used in neurosurgery for cavernous malformations or dorsal exophytic tumors are planned with the relationship between the pons, medulla, and cerebellar attachments in mind, and the same relationships guide interpretation of dorsal brainstem compression from Chiari I malformation or posterior fossa mass effect. This view keeps attention on the midbrain to medulla continuum and the cerebellar interface, the region that often determines obstructive hydrocephalus at the level of the fourth ventricle.
