Description
Viewed from the lateral aspect, the cerebral hemisphere dominates superiorly with clearly modeled gyri and sulci, while the cerebellar hemisphere sits posterior and inferior with folia-like striations along its surface. Inferior to the diencephalic region, the mesencephalon (midbrain) forms the superior segment of the brainstem, continuous caudally with the pons and then the medulla oblongata as the trunk tapers toward the spinal cord. The cerebellum lies dorsal to the pons and upper medulla, separated from the posterior cerebrum by the tentorial region. Proportions matter. This lateral perspective is where the midbrain-to-hindbrain junction becomes easiest to teach: the mesencephalon as a short connecting segment rostral to the pontine bulge, with the medulla narrowing toward the foramen magnum. For clinical neuroanatomy, that silhouette maps directly onto vascular territories and classic localization, for example basilar artery infarcts affecting the ventral pons with locked-in syndrome, or dorsal midbrain (Parinaud) syndrome from tectal compression in pineal region masses. It also supports surgical orientation for posterior fossa approaches where cerebellar retraction risks brainstem compression and postoperative respiratory compromise. Use this rendering in gross anatomy and neuroanatomy curricula when introducing the brainstem divisions (mesencephalon and hindbrain) in relation to the cerebrum and cerebellum, or in board-style review materials that emphasize lesion localization from surface anatomy.
