Emerging from the C7 intervertebral foramen, the C7 root courses laterally and slightly inferiorly in the lower neck, positioned posterior to the anterior scalene and approaching the interscalene interval where brachial plexus roots gather. From this anterior perspective, the nerve runs deep to the sternocleidomastoid and toward the clavicular region, where it contributes the full substance of the middle trunk. Adjacent yellow neural elements suggest neighboring C5 to T1 roots, while red cervical musculature frames the plexus as it moves from the vertebral column toward the shoulder girdle. Blue venous structures provide superficial and deep vascular landmarks. C7 anatomy matters because it sits at a practical crossroads for both diagnosis and regional anesthesia, with its fibers distributing broadly through the posterior cord and into the radial nerve, so deficits can present as mixed weakness in elbow extension, wrist and finger extension, and variable sensory change in the hand. This is the level where traction injuries and inflammatory radiculopathies often blur into plexopathy, and an anterior dissection style view helps clarify what lies anterior versus posterior to the scalene muscles when planning a supraclavicular or interscalene block. Small positional errors here have consequences. Use this plate in gross anatomy teaching of the neck and upper limb, in neurology or PM and R materials contrasting C7 radiculopathy with middle trunk lesions, and in anesthesia references that map needle trajectories to the brachial plexus roots and trunks near the clavicle. It also supports surgical orientation for anterior approaches to the lower cervical spine, where protecting ventral rami and nearby vessels is nonnegotiable.
