Description
Running along the medial aspect of the male leg, the great saphenous vein is shown superficial to the tibia, with tributaries converging toward it as they ascend from the distal leg. Posterolateral to this superficial channel, the circumflex fibular vein forms a curved loop around the superior fibula near the fibular head and neck, then courses medially toward deeper venous pathways. Calf venules and small perforating vessels bridge superficial and deep drainage. A directional arrow reinforces proximal venous return. Attention to the circumflex fibular vein is warranted because the region around the fibular head is a crowded surgical and sonographic neighborhood, where venous variants sit close to the common fibular (peroneal) nerve and the biceps femoris tendon insertion. For clinicians mapping superficial reflux or evaluating lateral knee swelling, a looping venous channel here can be mistaken for an atypical tributary of the great saphenous system or for a small varix unless its relationship to the proximal fibula is clear. This medial perspective also helps teach why perforator incompetence can redirect flow between compartments and why varicosities may present away from the expected saphenous track.
