Description

Spiraling around the proximal fibula, the circumflex fibular vein courses laterally near the fibular head and neck, then turns posteriorly to communicate with the veins of the lateral compartment of the leg. Proximally it lies inferior to the lateral femoral epicondyle and in close relationship to the tendon of biceps femoris and the insertion of fibularis (peroneus) longus, with the common fibular nerve tracking nearby around the fibular neck. Superficial tributaries from the lateral calf may converge in this region before draining into deeper venous channels that accompany the fibular (peroneal) artery and its venae comitantes. This proximal lateral leg focus matters because the fibular head is a high-traffic zone for vascular access, orthopedic exposure, and nerve vulnerability. During approaches for proximal fibula fractures, lateral meniscal or biceps femoris tendon procedures, and decompression around the fibular tunnel, small but consistent veins such as the circumflex fibular vein can be a source of bleeding and can be mistaken for larger named channels if the anatomy is unfamiliar. It also sits at a junction where superficial-to-deep venous communications can influence patterns of varicosities and the spread of superficial thrombophlebitis.
