Description
Oriented in left lateral profile, the thyroid cartilage sits superior and anterior to the complete ring of the cricoid, and their meeting point forms the paired cricothyroid joint with its synovial articulation between the inferior cornu of the thyroid and the lateral facet of the cricoid. Inferiorly, the cricoid continues into the first tracheal rings, while the cricothyroid membrane spans the interval between the inferior border of the thyroid cartilage and the superior border of the cricoid arch. Overlying red intrinsic musculature is visible, with the cricothyroid muscle coursing from the anterolateral cricoid to the inferior margin and horn of the thyroid, positioned anterior to the joint and superficial to the capsule. Small joint. Big consequences. Rotation and gliding at the cricothyroid joint adjust vocal fold length and tension by tilting the thyroid cartilage relative to the cricoid, the mechanical basis for pitch modulation and the target of cricothyroid muscle contraction. This view is the one you want when teaching recurrent laryngeal nerve injury patterns, because fixation or denervation of intrinsic laryngeal muscles changes arytenoid position, but impaired cricothyroid function from superior laryngeal nerve injury specifically blunts high-pitch phonation despite an otherwise mobile airway. The cricothyroid membrane shown just anterior to the joint also frames the anatomic window used for cricothyrotomy, where staying strictly midline avoids the joint laterally and reduces bleeding risk.
