Description
Viewed from the posterior male neck, the laryngeal framework is centered on the cricoid cartilage inferiorly and the thyroid cartilage superiorly, meeting laterally at the paired cricothyroid joints (synovial articulations between the inferior cornu of the thyroid cartilage and the cricoid lamina). Superior to the cricoid, the arytenoid cartilages sit on the posterior cricoid lamina, forming the cricoarytenoid joints and providing attachment sites for the posterior cricoarytenoid muscles. The epiglottis is seen superiorly in the midline, while the tracheal rings continue inferiorly from the cricoid, maintaining the airway axis. Posterior orientation matters because it clarifies how laryngeal motion is driven at the joints, not just by the vocal folds: rotation and gliding at the cricothyroid joint changes the anteroposterior relationship of thyroid and cricoid, increasing vocal fold length and tension for pitch elevation, while the posterior cricoarytenoid abducts the vocal folds through the arytenoids to open the rima glottidis. Small joint mechanics. Big consequences. Clinically, restricted cricothyroid mobility after laryngeal trauma, intubation injury, or inflammatory arthropathy can present as reduced upper range and vocal fatigue, and this same region sits adjacent to the cricothyroid membrane used for emergency cricothyrotomy, making precise landmark knowledge mandatory even when the operative window is anterior.
