Description
Anterior laryngeal anatomy is rendered with the thyroid cartilage positioned superior to the cricoid cartilage, and the paired cricothyroid joints located at their inferolateral margins where the inferior horn (cornu inferius) of the thyroid cartilage articulates with the lateral facet of the cricoid. The cricothyroid muscle lies on the anterolateral surface, its oblique and straight parts spanning from the anterolateral cricoid arch to the inferior border and inferior horn of the thyroid cartilage. Fibrous elements consistent with the cricothyroid membrane and adjacent laryngeal connective tissues frame the joint region. Small synovial recesses are implied by the smooth articular surfaces at the articulation. That synovial cricothyroid articulation is the mechanical hinge for pitch control, permitting rocking and slight gliding of the thyroid cartilage on the cricoid to lengthen and tense the vocal folds via the vocal ligaments. This is where laryngeal trauma can matter. Blunt anterior neck injury or iatrogenic strain can produce cricothyroid joint dysfunction, presenting with dysphonia and loss of upper register, and the joint’s position also provides a landmark context for emergency cricothyrotomy performed through the cricothyroid membrane just inferior to the thyroid cartilage.
