Description
Lateral anatomy of the male crus is presented with the crural (deep) fascia investing the lower leg musculature from just distal to the knee toward the ankle. Along the lateral compartment, the fascia overlies fibularis (peroneus) longus and brevis adjacent to the fibula, while posteriorly it encloses the triceps surae, gastrocnemius superficial to soleus, converging inferiorly on the calcaneal (Achilles) tendon. Anteriorly, tibialis anterior and extensor digitorum longus sit anterolateral to the tibia, and the fascia continues distally to blend with the extensor and fibular retinacula over the ankle and onto the dorsum and lateral border of the foot. Clean planes. Clear boundaries. Crural fascia matters because it is not just a covering, it defines compartments and sets the mechanical constraints that drive clinical problems. Acute compartment syndrome raises pressure beneath this inelastic sheath, most often in the anterior compartment after tibial shaft fracture, compromising the deep fibular nerve and anterior tibial vessels and producing pain with passive toe extension. Surgeons also use this lateral perspective to plan fasciotomy incisions, to orient relative to the fibular head and lateral malleolus, and to teach how intermuscular septa tether the fascia to tibia and fibula.
