Basilar Venous Plexus

Description

Centered on the clivus at the skull base, the basilar venous plexus forms a midline web of small venous channels lying on the basilar part of the occipital bone and dura, inferior to the dorsum sellae and posterior to the cavernous sinuses. Laterally, its emissary connections converge toward the cavernous sinus on each side and course posterolaterally into the superior petrosal sinus along the petrous ridge, establishing a venous crossroads between the middle and posterior cranial fossae. A cutaway or cross section context typically also places adjacent dural reflections in view, such as the tentorium cerebelli superior to the petrous temporal bone and the posterior clinoid region above the plexus. Basilar plexus anatomy matters because it explains how venous pressure, infection, or thrombosis can propagate across compartments when other routes are compromised. This is the collateral pathway neurosurgeons and neuroradiologists think about when evaluating cavernous sinus thrombosis, dural arteriovenous fistula drainage patterns, or posterior fossa venous outflow in the setting of jugular bulb stenosis or thrombosis.
