Description
Posterior cranial dura is presented with the occipital squama opened to expose the dural reflections of the posterior fossa. The tentorium cerebelli spans transversely, separating the supratentorial compartment from the cerebellar fossae, while the falx cerebelli descends in the midline inferior to the tentorium toward the foramen magnum. Along the internal occipital region, the dural layers converge around the venous sinus grooves, with the transverse sinuses running laterally and the midline occipital sinus tracking toward the confluence and down to the marginal sinus. Cervical alignment continues inferiorly into the upper vertebral canal, where the spinal dura maintains continuity at the foramen magnum. This posterior perspective matters because it places the tentorium and midline dural partitions in the same field as the dominant posterior fossa venous outflow pathways. It is the setup you want when teaching dural venous sinus thrombosis involving the transverse or sigmoid sinus, or when explaining why posterior fossa pressure can rapidly compromise brainstem and cerebellar drainage at the level of the confluence of sinuses. A key surgical orientation point. Approaches such as suboccipital craniotomy and posterior fossa decompression rely on safe dural opening relative to the midline occipital sinus and adjacent transverse sinus margins.
