Description
From an inferior perspective, both lungs are presented at their basal undersurfaces, emphasizing the broad diaphragmatic surface of each pulmonary organ and the overall contour of the bases. The right lung base appears wider and typically shows the inferolateral sweep of the costal surface, while the left base tapers slightly around the cardiac impression, with the lingular region lying anteromedial to the inferior lobe. Segmental and lobar divisions are organized around the bronchial tree, so the basal segments of the inferior lobes occupy most of the caudal field, bounded superiorly by the oblique fissures and, on the right, influenced by the horizontal fissure more anteriorly. Orientation at the hilum is implied even when not the focus, with the inferior pulmonary ligament descending from the root toward the base. Clean anatomy. Clinically, the diaphragmatic surface matters because dependent parenchyma is where pathology collects. Basal atelectasis after abdominal surgery, aspiration pneumonia in the posterior basal segments, and pleural effusions layering over the lung bases all map directly to this undersurface view, and the segmental layout helps explain why basilar crackles and dullness to percussion localize where they do. Bronchoscopic sampling and CT-guided procedures also rely on the basal segment nomenclature when describing lesions in the inferior lobes. The base is the reference.
