Seen from an anterior perspective, the right and left lungs are partitioned into color-coded bronchopulmonary segments, allowing the lobar outlines and segmental subdivisions to read clearly across the costal surfaces. On the right, the superior, middle, and inferior lobes are separated by the horizontal and oblique fissures, with segmental territories arranged around the expected course of the segmental bronchi. On the left, the superior and inferior lobes are divided by the oblique fissure, and the anterior segments of the upper lobe (including the lingular segments) occupy the anteroinferior portion adjacent to the cardiac impression. Segmental borders approximate the intersegmental planes that run between segmental veins. Segment-level anatomy matters when a process respects bronchial distribution. Aspiration pneumonia and endobronchial obstruction often localize to predictable bronchopulmonary segments, and segmental atelectasis on radiography or CT is interpreted against these divisions.
