Description

From a posterior perspective of the male ankle and foot, the calcaneus dominates inferiorly with the Achilles tendon (tendo calcaneus) descending from gastrocnemius and soleus to its insertion on the posterior calcaneal tuberosity. Medial and lateral to the tendon, the retromalleolar grooves frame the tendons of tibialis posterior, flexor digitorum longus, and flexor hallucis longus medially, and fibularis (peroneus) longus and brevis laterally as they course distally into the tarsus, metatarsus, and phalanges. Superiorly, the distal tibia and fibula anchor the scene, while the calcaneus and talus form the hindfoot scaffold; stabilizing ligamentous bands at the ankle and subtalar region are included. Blue neurovascular structures track behind the malleoli toward the plantar foot. Thick heel soft tissue stands out. Posterior ankle anatomy is where tendon pathology concentrates. A clear rendering of the Achilles tendon relative to Kager’s fat pad and the posterior calcaneus supports teaching around tendinopathy, partial rupture, and insertional enthesopathy, as well as operative planning for Achilles debridement or repair. The medial retromalleolar tunnel also matters clinically, since the tibial nerve and posterior tibial vessels share this confined space with the deep flexor tendons, the anatomic setup for tarsal tunnel syndrome and for safe exposure during hindfoot surgery.
