Description
Arising from the inferior mental spines of the mandibular symphysis, the paired geniohyoid muscles form narrow, strap-like bands that run posteriorly and slightly inferiorly toward the body of the hyoid bone, sitting immediately superior to the mylohyoid and medial to the anterior belly of digastric. From an inferior perspective, the mandibular teeth and alveolar processes frame the anterior field, while the hard palate and choanae lie superior and posterior, orienting the floor of the mouth against the upper cervical spine. The hyoid sits inferior to the mandible and anterior to the cervical vertebral bodies. Midline anatomy is easy to judge. This angle matters because geniohyoid is one of the main suprahyoid muscles that elevates and draws the hyoid anteriorly during the pharyngeal phase of swallowing, and it can be selectively targeted in submental ultrasound, EMG placement, and dysphagia rehabilitation planning. Surgeons working in the submental and submandibular spaces use the relationship between geniohyoid, mylohyoid, and the mandibular symphysis to orient dissection planes and avoid misidentifying the muscle layer when approaching ranulas or performing genioglossus and hyoid suspension procedures.
