Description
Centered in an anterior view, the paired hip bones (os coxae) meet at the pubic symphysis, with the iliac crests flaring superiorly and laterally to frame the pelvic inlet. Each acetabulum faces anterolaterally, receiving the femoral head, while the obturator foramen lies inferior and medial to the acetabulum, bordered by the pubis anteriorly and the ischium posteroinferiorly. Posteriorly in the midline, the sacrum and coccyx align with the pelvic ring, and the L5 vertebra crowns the sacral promontory. Male pelvic proportions are evident, with a narrower subpubic angle and a more heart-shaped inlet than typically seen in the female pelvis. Anterior pelvic osteology matters because it anchors how clinicians think about load transfer from the lumbar spine through the sacroiliac joints to the hip joints and lower limbs. The relationship of the acetabulum to the obturator foramen and pubic rami is a working map for acetabular fracture patterns, anterior column fixation, and safe corridors for screws placed along the pelvic brim.
