Description
Seen from an inferior perspective, the suboccipital surface of the cerebellar hemispheres and vermis is crossed by the inferior cerebellar veins as they converge toward the midline and laterally toward the petrosal and occipital dural sinuses. The brainstem occupies the central field, with the medulla oblongata continuous superiorly with the pons, and the cerebellar tonsils sit posteroinferior to the medulla near the foramen magnum region. Cranial nerves emerge from the ventral brainstem and cerebellopontine angle, their roots running anterolaterally relative to the cerebellar veins and adjacent vascular channels. Venous anatomy on the inferior cerebellar surface matters because it is the territory most often encountered when working around the posterior fossa, where small bridging veins can be the difference between a clean corridor and troublesome bleeding. In far-lateral or retrosigmoid approaches, veins draining the cerebellar tonsil and inferior hemispheric surface may tether the cerebellum to the dura near the sigmoid and inferior petrosal sinus region, limiting retraction and increasing the risk of venous infarction if sacrificed. Venous drainage patterns also frame how you interpret posterior fossa vascular malformations and dural arteriovenous fistulas, where retrograde flow can recruit inferior cerebellar veins and produce edema or hemorrhage in the tonsillar region.
