Positioned on the inferior surface of the cerebellar hemispheres, the inferior cerebellar veins course posteriorly and medially along the cerebellar folia toward the vermis, then sweep toward the midline venous collectors at the cranial base. From the rear, their channels can be followed as they converge on the petrosal and occipital venous pathways near the foramen magnum, in proximity to the cerebellar tonsils and the dorsal medulla. Deep to the cerebellar cortex, tributaries communicate with veins around the fourth ventricle and brainstem, while superiorly the venous field transitions toward the straight sinus and vein of Galen territory. Blue venous trunks and red arterial counterparts provide orientation against the cerebellar gyri and sulci. Posterior venous anatomy matters when the surgical corridor approaches the posterior fossa, because venous sacrifice along the inferior cerebellar surface can precipitate venous infarction, edema, or hemorrhage in a tight compartment. This angle also clarifies why lesions near the cerebellar tonsils and vermis, including dural arteriovenous fistulas draining toward the transverse-sigmoid complex or the inferior petrosal system, often present with congestive posterior fossa symptoms and can complicate suboccipital exposure. Small veins, big consequences. The relationship to the dorsal brainstem and upper cervical cord helps frame venous outflow patterns relevant to raised intracranial pressure and posterior fossa crowding.
