Description

Arising from the anterior aspect of the abdominal aorta at approximately the L3 level, the inferior mesenteric artery (IMA) is traced posteriorly as it descends toward the left lower quadrant, running retroperitoneally before entering the sigmoid mesocolon. From this vantage, the left colic artery courses superiorly and laterally toward the descending colon, while paired sigmoid arteries sweep inferiorly within the mesenteric fold to the sigmoid colon, and the superior rectal artery continues distally into the pelvis. The descending and sigmoid colon sit left of midline, their haustra and appendices epiploicae outlined along the antimesenteric border, with the mesentery fanning medially to the posterior abdominal wall. Adjacent venous drainage is depicted in parallel, consistent with the inferior mesenteric vein tracking superiorly toward the portal system. Clean landmarks. Posterior visualization of the IMA territory matters when you are teaching hindgut perfusion and the collateral pathways between the superior and inferior mesenteric circulations, including the marginal artery of the colon and the arc of Riolan. This is the operative field for left colectomy and sigmoid resection, where high versus low ligation of the IMA at its origin (near the inferior mesenteric plexus) can affect perfusion to the distal colon and contribute to anastomotic leak risk if collateral flow is limited. The same arterial map frames ischemic colitis patterns, which often involve watershed zones near the splenic flexure and rectosigmoid junction. Ideal for gross anatomy lab review of posterior abdominal wall relationships, GI surgery teaching files on oncologic ligation planes, and colorectal textbook figures explaining IMA branch patterns and mesenteric folds.
