Description
Arising from the tibial nerve in the popliteal fossa, the inferomedial genicular nerve is traced on the posterior aspect of the knee as it courses distally and medially along the proximal tibia, then turns to ascend toward the inferomedial joint capsule. Behind the femoral condyles and tibial plateau, the nerve runs close to the posterior capsule and the capsular reflections, with its articular branch approaching the medial tibiofemoral compartment deep to the soft tissues of the pes anserinus region. Articular cartilage is rendered on the opposing surfaces of the distal femur and proximal tibia, emphasizing the joint line the nerve ultimately targets. Orientation is posterior. Posterior anatomy matters for knee pain workups and interventions because genicular sensory innervation is often approached from the back when mapping pain generators or when anterior scarring limits access. The inferomedial genicular nerve is a frequent target in genicular nerve blocks and radiofrequency ablation for osteoarthritis related pain, and its proximity to the tibial collateral ligament attachments and the medial tibial flare helps explain why needle placement can drift off the capsular plane and fail to capture the articular branch. For surgeons, this course also frames where iatrogenic traction or cautery injury may occur during posteromedial approaches and hamstring tendon harvest, producing medial joint line dysesthesia.
