Description
Centered within the posterior thorax, the right and left lungs are opened to reveal the intrapulmonary bronchial tree arising from the trachea and main bronchi, then branching into lobar and segmental bronchi that course inferiorly and laterally toward the peripheral parenchyma. Segmental anatomy is implied by the orderly, tree-like distribution of bronchi within each lung, with the right lung typically subdividing into superior, middle, and inferior lobes and the left lung into superior and inferior lobes. Medial to the lungs sits the heart, with the great vessels entering and leaving the mediastinum, while fine terminal airways and clustered alveoli occupy the most distal regions. Green lymphatic vessels and hilar or tracheobronchial lymph nodes track along bronchovascular pathways. Posterior orientation matters because it mirrors what you evaluate on back-focused thoracic imaging and during posterolateral thoracotomy, where the relationship of segmental bronchi to adjacent vessels guides safe dissection.
