Positioned in the right upper quadrant, the liver occupies the subdiaphragmatic space beneath the right hemidiaphragm and deep to the right costal margin, with a smaller left lobe extending medially across the epigastrium. A lateral, slightly oblique standing view places the hepatic mass anterior to the right kidney and suprarenal gland, and inferior to the cardiophrenic region of the thorax. Surface topography aligns the superior hepatic dome with approximately the 5th intercostal space at the midclavicular line, while the inferior margin approaches the right costal arch in quiet respiration. Orientation is clear. This perspective matters when you need to relate hepatic anatomy to the body wall, since physical examination and many procedures start with surface landmarks rather than cross sectional imaging. The inferior liver edge becomes palpable with hepatomegaly from congestive heart failure, acute hepatitis, or infiltrative disease, and this oblique view helps explain why right upper quadrant tenderness can be accentuated with deep inspiration during palpation. Interventional planning also depends on appreciating the liver’s cranio-caudal excursion with breathing, a practical consideration for percutaneous liver biopsy and for ultrasound guided assessment of focal hepatic lesions along the anterior axillary to midaxillary windows. Use this asset in gross anatomy and clinical skills teaching to connect hepatic (hepar) location with inspection, palpation, and percussion, and in patient education materials explaining abdominal pain patterns or hepatomegaly.
