Description
Centered in the right upper quadrant, the liver sits immediately inferior to the right hemidiaphragm and deep to the lower costal margin, with its superior surface conforming to the diaphragmatic dome. A frontal, standing external perspective places the hepatic mass predominantly to the right of the median plane, while the left lobe extends medially and slightly leftward toward the epigastrium. From this orientation, the anterior surface relates to the anterior abdominal wall, and the inferior border tracks obliquely toward the right costal margin. Surface anatomy matters. Clinically, this spatial relationship explains why diaphragmatic irritation from hepatic capsular distension can refer pain to the right shoulder via the phrenic nerve, and why deep inspiration can bring the inferior liver edge into reach during palpation. The view also supports teaching the difference between true hepatomegaly and a low-lying liver (for example in hyperinflated lungs), where the liver descends without necessarily increasing in volume. Position relative to ribs and diaphragm drives both physical exam findings and procedural planning.
