Description
Rendered on an anterior view of an adult male thorax with partial transparency, the trachea descends in the midline and bifurcates at the carina into the right and left main bronchi, each entering the lung at the hilum. Both lungs occupy their pleural cavities lateral to the mediastinum, with the right lung broader and positioned slightly higher than the left, and the pleural reflections extending inferiorly toward the costodiaphragmatic recesses. Pleural effusion is indicated by dependent fluid layering along the inferior and lateral pleural spaces, blunting the normally acute costophrenic angles. Clinically, anterior visualization of pleural fluid helps teach why even moderate effusions preferentially collect in the posterior and inferior thorax in the upright patient yet still alter the contour of the basal pleura and compress adjacent lung parenchyma. The blunted costophrenic angle is the classic plain radiograph sign, and correlates with reduced basilar expansion and a restrictive pattern on exam, with decreased breath sounds and dullness to percussion over the affected hemithorax. For procedural planning, this anatomy underpins thoracentesis site selection in the midaxillary line while staying superior to the rib to avoid the intercostal neurovascular bundle.
