Description
Arising from the profunda femoris artery in the proximal thigh, the medial circumflex femoral artery courses posteriorly and medially toward the hip, wrapping around the posteromedial aspect of the femoral neck at the base of the head. From a posterior perspective, its deep trajectory is read against the bony landmarks of the pelvis and proximal femur, with the acetabulum, femoral head, and neck forming the immediate spatial reference. Superiorly, the lower lumbar vertebrae and sacrum anchor the midline, while laterally the ilium and ischium frame the posterior hip. Red arterial channels and paired blue venous companions track along the pelvic brim and into the upper thigh. Deep and unforgiving. This posterior view matters because the medial circumflex femoral artery is the dominant source of the retinacular vessels to the femoral head in the adult, and those branches are vulnerable where they run close to the capsule and neck. In displaced femoral neck fractures, traumatic hip dislocation, or during posterior approaches to the hip (including total hip arthroplasty), compromise of this vessel can precipitate osteonecrosis of the femoral head and delayed collapse. Seeing the artery in relation to the posterior acetabular rim and proximal femur clarifies why seemingly small changes in neck alignment or capsular disruption can have outsized vascular consequences.
