Description
Posteriorly, the medial circumflex femoral vein courses in the deep gluteal and proximal posterior thigh region, curving around the posteromedial aspect of the proximal femur near the intertrochanteric crest and adjacent hip capsule. Running in close company with the medial circumflex femoral artery, the vein lies deep to gluteus maximus and tracks toward the pelvis to join the profunda femoris system, with the femoral shaft positioned inferiorly and laterally as the main bony reference. Portions of the lumbar vertebrae and os coxae frame the superior field, orienting the viewer to the posterior pelvis and upper thigh. Small venules and accompanying vascular branches populate the periarticular network around the hip. This posterior perspective matters because the medial circumflex femoral vessels form the dominant vascular pedicle to the femoral head and neck via retinacular branches, and their course places them at risk during posterior approaches to the hip. A short vessel with high stakes. Iatrogenic injury during total hip arthroplasty, acetabular fracture fixation, or posterior capsulotomy can contribute to hemorrhage and, when the arterial side is compromised, avascular necrosis of the femoral head, so clear orientation to the vessel’s relationship to the posterior proximal femur and capsule is more than academic.
