Description
Posteriorly, the medial circumflex femoral vein tracks through the proximal medial thigh as venae comitantes alongside the medial circumflex femoral artery, passing deep to the femoral neck and toward the posterior aspect of the hip. Relative to the pelvis, the vessel course lies inferior to the acetabulum and adjacent to the ischium, with the sacrum centered in the background and the paired ilia forming the superior bony frame. Muscle planes provide the context, with iliacus and psoas major superior and anterior at the pelvic brim and the proximal quadriceps (including rectus femoris) positioned more anteriorly, while deeper short external rotators would logically overlie portions of the circumflex system from this perspective. A posterior rendering of the medial circumflex femoral vein matters because it maps the venous counterpart of the dominant vascular pedicle to the femoral head and neck, an area where small-vessel injury can have outsized consequences. In femoral neck fracture patterns and posterior hip approaches, disruption of the medial circumflex femoral artery is the classic concern, but the paired venous channel is often the structure that declares itself intraoperatively through brisk bleeding and difficult-to-control oozing in the deep interval.
