Medial habenular nucleus sits in the dorsal epithalamus, immediately medial to the lateral habenular nucleus and just superior to the midbrain tectum in a posterior-oriented view. The paired nuclei flank the roof of the third ventricle near the pineal recess, with the habenular commissure bridging the midline posterior to the columns of the fornix and adjacent to the posterior aspect of the thalamus. Stria medullaris thalami converges onto the habenula from anterosuperior, underscoring the nucleus as a relay between limbic forebrain inputs and brainstem targets through the fasciculus retroflexus (habenulointerpeduncular tract).
