Description
Arising as a terminal branch of the tibial nerve distal to the tarsal tunnel, the medial plantar nerve courses anteriorly and inferiorly along the medial longitudinal arch, deep to the abductor hallucis and then between abductor hallucis and flexor digitorum brevis. Medially, its path parallels the medial plantar artery as it passes toward the navicular and medial cuneiform region, then continues distally toward the metatarsals. Plantar digital branches track toward the hallux and adjacent toes, with smaller fasciculi and the neurilemma implied by the labeled nerve trunk. Bony landmarks such as the medial malleolus, talus, calcaneus, navicular, and the first metatarsal provide orientation for the nerve’s proximal to distal course. Medial exposure of this nerve matters because symptoms along the medial sole and plantar aspect of the hallux often trace back to entrapment beneath the abductor hallucis fascia or at the level of the tarsal tunnel, where the tibial nerve divides into medial and lateral plantar nerves. A common teaching point is the distinction from Baxter nerve entrapment (inferior calcaneal nerve) that produces more lateral plantar heel pain, while medial plantar neuropathy can mimic plantar fasciitis yet presents with sensory changes along the medial forefoot. This is a tight corridor.
