Description
Arising as a terminal branch of the tibial nerve posterior to the medial malleolus, the lateral plantar nerve tracks anteriorly and laterally across the plantar aspect of the foot, deep to the flexor retinaculum and along the medial surface of the calcaneus before angling toward the midfoot. From this medial perspective, the nerve’s oblique course can be followed as it crosses the sole in proximity to the plantar venous network and the bony landmarks of the talus, calcaneus, cuboid, metatarsals, and phalanges. Proximally, the tibia and fibula frame the distal leg, while distally the nerve’s path aligns with the longitudinal arch as it approaches the lateral forefoot for cutaneous and intrinsic muscle innervation. Spatial relationships matter here. Clinically, this angle helps clarify why symptoms from tarsal tunnel syndrome can extend beyond the medial plantar distribution, since tibial nerve compression beneath the flexor retinaculum may compromise the lateral plantar nerve before it fans into superficial and deep branches. The illustration also supports surgical planning for medial approaches around the calcaneal tunnel and plantar fascia, where retraction near the neurovascular bundle risks iatrogenic neurapraxia or neuroma formation.
