Description

Arising from the posterior division of the mandibular nerve (V3), the auriculotemporal nerve is traced within the infratemporal fossa as it courses posterior to the neck of the mandible and ascends toward the temporal region. Fine roots classically encircle the middle meningeal artery near the foramen spinosum before the nerve runs superiorly, deep to the lateral pterygoid and then between the temporomandibular joint capsule and the parotid gland. Distally, it distributes as sensory branches to the tragus and auricle, the external acoustic meatus, the temporal scalp, and the TMJ, while communicating with the facial nerve in the parotid region. Small structures, tight spaces. This trajectory matters because the auriculotemporal nerve is a frequent source of peri-auricular pain and dysesthesia after parotidectomy, temporomandibular joint surgery, or mandibular condylar trauma, where traction or scarring can irritate the nerve as it lies just posterior and medial to the condylar neck. The same anatomy explains Frey syndrome, with aberrant parasympathetic fibers from the otic ganglion hitchhiking along the auriculotemporal nerve and reinnervating sweat glands over the parotid bed, producing gustatory sweating and flushing. For regional anesthesia, its superficial temporal course just anterior to the ear provides a practical target for an auriculotemporal nerve block in temporal scalp laceration repair or chronic TMJ pain management.
