Description
Centered in the mediastinum, the male heart is rendered in an external (epicardial) view with fine cardiac lymphatic vessels tracking along the coronary sulci and converging toward the roots of the great vessels. Lymphatic channels accompany the coronary arteries and cardiac veins over the anterior surfaces of the right and left ventricles, then course superiorly around the ascending aorta and pulmonary trunk toward pericardial and tracheobronchial lymph nodes near the carina. Green lymphatics and nodal stations sit in close topographic relationship to the superior vena cava, aortic arch region, and pulmonary arteries, while cardiac nerves (yellow) descend from the cervical and thoracic cardiac plexuses to the atria and ventricular bases. Spatially, the densest lymphatic network lies superficial to the myocardium, deep to the fibrous pericardium, and clustered at the arterial poles. Cardiac lymphatic anatomy matters when myocardial inflammation, infarction, or cardiac surgery triggers pericardial and mediastinal lymphadenopathy or altered lymph drainage. Surgeons working near the aortopulmonary window, pulmonary artery bifurcation, or along the superior vena cava routinely encounter these nodal basins, and their relationship to the phrenic nerve and cardiac plexus can influence dissection planes and postoperative complications.
