Description
Prominent in the posterior neck and upper thorax, the longissimus capitis is positioned lateral to semispinalis capitis and medial to the more superficial splenius capitis, running superolaterally from the upper thoracic and lower cervical transverse processes toward the mastoid region. Superiorly, its fibers approach the temporal bone near the mastoid process, while inferiorly the muscle blends with the longissimus cervicis within the erector spinae mass adjacent to the cervical vertebrae. Surrounding osseous landmarks include the occiput and mandible of the skull, the cervical spinous processes, and the posterior elements of the shoulder girdle including scapula and clavicle. A posterior oblique vantage also frames the sternocleidomastoid along the anterolateral neck, helping orient the viewer to depth and compartment. Clinically, this view is most helpful for separating the longissimus capitis from neighboring posterior cervical extensors when teaching palpation, trigger point referral patterns, and the muscular contributions to ipsilateral head rotation with extension. Strain and overuse in the longissimus capitis often accompanies whiplash type acceleration injuries, and the muscle is frequently implicated in cervicogenic headache patterns that localize to the occipital and mastoid regions. Distinguishing it from semispinalis capitis and splenius capitis matters when interpreting posterior neck pain and when planning posterior cervical approaches where the dissection plane follows natural fascial intervals.
