Description
Running distally from the tibial nerve in the tarsal tunnel, the medial plantar nerve courses deep to the flexor retinaculum and enters the plantar aspect of the foot, tracking anteriorly between the abductor hallucis and flexor digitorum brevis. Nearby osseous landmarks include the distal tibia and fibula at the ankle mortise, the talus superior to the calcaneus, and the tarsals and metatarsals arranged distally toward the phalanges. Medially, the nerve relates to the calcaneal region and the arch, then continues toward the medial metatarsals where its digital branches distribute toward the hallux and adjacent toes. Yellow nerve rendering contrasts against the bony framework for clear topographic orientation. Clinically, this is the nerve most often implicated in medial plantar neuropathy, sometimes called jogger’s foot, from repetitive compression in the proximal medial arch under the abductor hallucis. Pain and paresthesia localize to the medial sole and plantar aspects of the first through third toes, and weakness may involve the abductor hallucis, flexor hallucis brevis, and flexor digitorum brevis depending on the level of entrapment. It matters in tibial nerve block planning and in differentiating proximal tarsal tunnel syndrome from more distal branch entrapment when symptoms spare the lateral plantar territory. A practical map.
