Description
Venous drainage of the small intestine is presented from the level of the intestinal wall outward, tracing venules in the submucosa as they converge into larger tributaries of the portal venous system. From the duodenum, jejunum, and ileum, straight vessels (venae rectae) run within the mesentery and join successive venous arcades before collecting into the superior mesenteric vein, which courses superiorly and to the right toward the hepatic portal circulation. Proximal tributaries sit in close company with their paired arteries and lymphatics along the mesenteric border, while more distal venous channels show increasing caliber and fewer anastomotic loops. Portal venous anatomy matters because the small bowel is the dominant inflow source to the liver, and its venous pattern influences both disease spread and operative planning. Mesenteric venous thrombosis often begins in smaller venous radicles and propagates centrally, so appreciating the arcade-to-trunk pathway helps correlate segmental ischemia on CT venography with the likely level of occlusion. Surgeons also think in these terms during small bowel resection, where high ligation near the superior mesenteric vein risks bleeding and can compromise adjacent venous drainage if the arcades are taken too proximally. Bleeding here is unforgiving. Suitable for teaching gastrointestinal vascular anatomy in gross anatomy and GI blocks, this illustration also supports figures in hepatology and portal hypertension chapters where portosystemic hemodynamics depend on mesenteric inflow.
