Seen from a posterior perspective, the right tibia and fibula form the bony scaffold for the deep posterior compartment while the posterior tibial artery descends from the popliteal region and tracks inferiorly along the posterior surface of the leg. Just distal to the popliteus, its course relates to the soleal line of the tibia, then continues deep to the soleus as it runs toward the medial malleolus. Flanking venous channels accompany the artery, with superficial veins positioned more posterior and subcutaneous than the deep pair. Orientation is clear. Following the posterior tibial artery below the soleal line matters because this is the segment commonly approached when teaching distal runoff, compartment anatomy, and relationships that guide vascular access and surgical exposure. In acute limb ischemia and peripheral arterial disease, patency of the posterior tibial artery helps predict wound healing potential in the heel and plantar foot, and its distal pulse is a standard bedside target posterior to the medial malleolus. During deep posterior compartment release for compartment syndrome, keeping the artery’s expected medial trajectory in mind helps reduce iatrogenic injury while decompressing the flexor compartment.
