Description
Running down the anterior compartment of the male leg, the anterior tibial artery arises from the popliteal artery at the inferior border of the popliteus and passes through the proximal opening in the interosseous membrane to reach the anterior surface of the leg. It then descends on the interosseous membrane between tibialis anterior medially and extensor digitorum longus laterally, with the tibia positioned medially and the fibula laterally as constant bony references. At the ankle joint, the vessel crosses anterior to the talocrural articulation and continues distally as the dorsalis pedis artery on the dorsum of the foot. Companion deep veins (venae comitantes) track alongside the artery, and the patella caps the proximal view above the tibial plateau. Clinically, this anterior projection clarifies where the anterior tibial artery is at risk during proximal tibial and fibular procedures and why loss of a dorsalis pedis pulse can reflect disease proximal to the ankle, not just in the foot. A frequent teaching point sits at the distal transition: palpation of the dorsalis pedis just lateral to the tendon of extensor hallucis longus, and compression risks in anterior compartment syndrome where rising fascial pressures compromise the anterior tibial artery and deep peroneal nerve. Bone landmarks make the course easy to triangulate.
