Description

Positioned posterior to the heart, the right and left pulmonary veins course from the lung hila to enter the left atrium, crossing the posterior mediastinum just anterior to the esophagus and descending thoracic aorta. Superior and inferior veins on each side can be inferred, with the right pulmonary veins running slightly longer and more obliquely because the left atrium sits left of midline. Branching venules converge within each lung from the capillary beds surrounding the alveoli, then travel medially alongside the bronchi and pulmonary arteries toward the hilum. Color coding distinguishes pulmonary veins from the pulmonary arteries and highlights their return route into the cardiac chamber. Clear relationships. A posterior view puts the pulmonary venous ostia and left atrial inflow in the same field, which is exactly what you want when teaching pulmonary venous anatomy or planning left atrial procedures. Catheter ablation for atrial fibrillation often targets tissue around the pulmonary vein antra, and variant venous anatomy (a common left pulmonary venous trunk or an accessory right middle pulmonary vein) changes lesion sets and risk to adjacent structures. The proximity to the esophagus also matters, since posterior left atrial energy delivery carries the rare but feared complication of atrioesophageal fistula. Faculty can drop this into cardiopulmonary blocks to connect alveolar gas exchange to the venous return of oxygenated blood, and radiology educators can pair it with contrast-enhanced CT or MR angiography of the pulmonary veins in the posterior mediastinum. Cardiothoracic teams may also use it for patient-facing explanations before pulmonary vein isolation or repair of partial anomalous pulmonary venous return.
