Description
Posterior to the heart, the right superior and right inferior pulmonary veins converge from segmental venules within the right lung and course medially to enter the left atrium. Their ostia sit on the posterior wall of the left atrium, just inferior to the transverse pericardial sinus and anterior to the esophagus, while the right pulmonary artery lies superior and slightly anterior at the hilum. From this angle, the venous trunks are read against the posterior atrial contour, with the right atrium and venae cavae positioned more to the viewer’s right and anterior. Scale and spacing matter here. Posterior pulmonary venous anatomy is the map for electrophysiology and thoracic surgery. Pulmonary vein isolation for atrial fibrillation targets the antra around these veins, and variants such as a common right pulmonary venous trunk or an additional middle lobe vein change both lesion sets and the risk profile for pulmonary vein stenosis. Surgeons also reference these relationships during right lung resections and mediastinal dissection, where traction or stapling near the left atrial cuff can cause brisk bleeding and where the close posterior relationship to the esophagus frames the risk of atrioesophageal fistula after ablation.
