Description
Arising most often from the proximal anterior cerebral artery at the A1 to A2 junction, the recurrent artery of Heubner is traced from an inferior (basal) view as it courses posteriorly and laterally toward the anterior perforated substance. From this perspective the vessel sits anteromedial to the optic chiasm and optic tracts, then turns back along the medial aspect of the frontal lobe base near the lamina terminalis and gyrus rectus. The internal carotid artery bifurcation, anterior communicating artery complex, and proximal middle cerebral arteries provide the local arterial context, while the brainstem lies posterior and midline with cranial nerves emerging from the interpeduncular and pontomedullary regions. Small perforators penetrate superiorly from the basal surface toward deep gray nuclei. Heubner’s artery matters because it supplies the head of the caudate nucleus, anterior limb of the internal capsule, and adjacent putamen and globus pallidus, territories where a small infarct can produce dense contralateral motor weakness and dysarthria out of proportion to lesion size. Aneurysms of the anterior communicating artery and surgical or endovascular work at the A1 segment place this vessel at risk, and its variable origin and caliber make it a common “missed” perforator when teaching the anterior circulation. One slip here, and the internal capsule pays.
