Prominent costae (ribs) arc inferolaterally from their posterior articulations with the thoracic vertebrae toward the anterior midline, where costal cartilages converge on the sternum. Superiorly, the manubrium aligns with the clavicular notches, while the sternal body receives the true ribs (typically 1 through 7) along the costal margins. False ribs (8 through 10) angle anteroinferiorly to form the costal arch, and the floating ribs (11 and 12) terminate freely in the lateral abdominal wall. Midline continuity continues inferiorly into the lumbar vertebrae and sacrum, with the iliac crests and pelvic inlet framing the lower boundary of the field. Anterior rib anatomy matters because surface landmarks and bony geometry guide both imaging interpretation and procedural safety in the thorax. The sternal angle (Angle of Louis) corresponds to the 2nd costal cartilage, the standard starting point for counting ribs on physical exam and for localizing intercostal spaces on radiographs and CT. Needle decompression for tension pneumothorax and chest tube placement hinge on reliable rib and intercostal spacing, and the relationship of the costal margin to the xiphoid process becomes relevant when evaluating costochondral injury, rib fractures, and flail chest patterns.
