Description
Running along the medial aspect of the male lower limb, the saphenous nerve is traced as the terminal cutaneous branch of the femoral nerve, descending from the distal thigh toward the medial leg and medial margin of the foot. Proximally it travels near the femoral vessels in the adductor canal, then becomes superficial around the medial knee to course anterior to the medial malleolus. The tibia, fibula, patella, and tarsal and metatarsal skeleton provide bony landmarks, while adjacent superficial veins, commonly including the great saphenous vein, map the neurovascular corridor. Spatially, the nerve remains medial and subcutaneous distally. Clear landmarks. Clinically, this view helps you teach and plan for purely sensory symptoms along the anteromedial leg and medial foot, where saphenous neuropathy can follow knee arthroscopy, hamstring tendon harvest, or trauma at the medial tibial plateau. The close relationship to the great saphenous vein matters during venous cutdown, varicose vein procedures, or endovenous ablation, where inadvertent nerve irritation can produce burning dysesthesia without motor deficit. The depiction around the medial malleolus also supports discussions of ankle incisions and the differential diagnosis of medial foot numbness compared with tibial nerve or superficial fibular nerve territories.
