Description
Running from the lumbar plexus to the medial leg, the saphenous nerve is traced in anterior projection as it enters the thigh within the adductor canal, then becomes superficial between the sartorius and gracilis to accompany the great saphenous vein along the anteromedial tibia. Proximally, the pelvis and femur establish the nerve’s course relative to the knee joint, with the tibia and fibula forming the bony corridor of the leg and the tarsal and metatarsal bones continuing the skeletal framework distally. Blue venous drainage parallels the nerve, reinforcing the well known relationship between the saphenous nerve and the great saphenous vein from the medial knee toward the medial malleolus. Clear landmarks. Clinically, this anatomy matters for medial leg sensory symptoms and iatrogenic injury. The saphenous nerve can be irritated or transected during great saphenous vein harvesting for coronary artery bypass grafting and during varicose vein procedures, producing dysesthesia along the medial tibial border without motor deficit. It also frames safe technique for an adductor canal (saphenous) nerve block, where anesthetic spread within the canal can relieve anteromedial knee pain while largely preserving quadriceps strength compared with femoral nerve block.
