Centered in the left upper quadrant of the abdomen, the stomach sits inferior to the diaphragm and left lobe of the liver, with the distal esophagus entering at the cardia just left of the midline. The fundus rises superiorly beneath the left hemidiaphragm, while the body curves inferomedially toward the antrum and pylorus. Laterally, the greater curvature drapes toward the left abdominal wall; medially, the lesser curvature faces the liver and the expected course of the lesser omentum. An upright, transparent-body presentation preserves external surface anatomy while localizing the upper gastrointestinal viscera. Clear orientation. Clinically, this localization matters when correlating epigastric and left upper quadrant pain with gastric pathology, from peptic ulcer disease along the lesser curvature to gastric outlet obstruction at the pylorus. The esophagogastric junction is a key teaching point for gastroesophageal reflux disease and hiatal hernia, where proximal stomach can displace superiorly through the esophageal hiatus and alter the normal relationship to the diaphragm.
